If you schedule an appointment with Flourish True, your name, e-mail address and phone number will be collected directly from you. Your name, e-mail address or phone number may be used to contact you after your call with follow-up information.

If you become a client or customer, Flourish True will collect your name, email, address, and phone number and other information from you directly for use to provide services to you. We may also obtain information from a governmental agency or registry or other professionals or consultants who serve you. You will also be requested to sign up for the client portal so that any communications and services that are provided are sent and protected.

If you retain Flourish True you have the option to pay online through Practice Better which integrates the Square payment system who completes the purchase transaction. You agree that you gave this information voluntarily for the contents that you received. By using this Website, you agree not to hold Flourish True responsible for any security breach and for any unauthorized use of your personal information by third parties.

  • Services. To provide dietitian or wellness services (as applicable) to you.

  • Other business purposes. Your information may be used for data analysis, determine the effectives of promotional campaigns, and to evaluate and improve services, products, and marketing.

For example: when you consent to the disclosure, when we are required or permitted by law to do so, as required by the College of Dietitians of Ontario, to services providers, including organizations or individuals retained by us to perform functions on our behalf, such as payment processing, e-mail deliver, hosting services, document management, office services, data processing, scheduling, collection of outstanding debts, a financial institution (on a confidential basis and solely in connection with the assignment to receive payment, the provision of security or other financial arrangements, in the event of a merger or growth by us, if we retain services on your behalf or to a person who, in the reasonable judgement by us, is providing or seeking information as your agent.

College of Dietitians of Ontario

*College of Dieticians of Ontario is the regulatory body for any Registered Dietitian practicing in Ontario. The College can choose to audit or investigate any Registered Dietitian at any time. This may involve disclosure of client information depending on the grounds of inquiry that the College is making.

How long will you store my information?

Your information is stored for as long as necessary as set out in our Privacy Policy, unless otherwise required or permitted by law (including, without limitation, tax, accounting, legal or College of Dietitians of Ontario obligations). If you become a client at Flourish True and dietetic services are rendered, records must be retained for 10 years in accordance with the College of Dietitians of Ontario Record Keeping Guidelines.
